The testosterone-to-cortisol ratio improvements observed in the Fernández-Landa research hold special significance for women. While women produce lower absolute levels of testosterone than men, maintaining optimal testosterone-to-cortisol ratios remains crucial for muscle protein synthesis, recovery, and training adaptations. The improvement in this ratio when combining HMB with creatine suggests realizable benefits for female athletes dealing with high training stress.
Intramuscular Fat Reduction: A Hidden Benefit
Women concerned about muscle quality should also be interested in research showing HMB’s effects on intramuscular adipose tissue (IMAT). A study with middle-aged women found that HMB plus vitamin D supplementation reduced IMAT by 36% compared to placebo, even in sedentary participants.[19]
This reduction in intramuscular fat infiltration occurred alongside resistance training benefits, but remarkably, the IMAT improvements happened even without exercise. The Critical HMB + Vitamin D Connection

Although resistance training still increased muscle volume in both the placebo group and the HMB + vitamin D group, the combination of HMB + vitamin D caused substantial reductions in IMAT volume that exercise did not achieve. Importantly, HMB + D did this even in the sedentary condition.[19]
This finding has profound implications for female athletes, as 60-75% of women have insufficient vitamin D status, and quite frankly, aren’t getting enough sunshine.
A follow-up study published in 2020 confirmed that combined HMB and vitamin D supplementation improved functional performance even in non-exercising older adults, highlighting the synergistic relationship between these nutrients.[22]
For this reason, we’ve included a reasonable 400IU of vitamin D3 as cholecalciferol, explained a bit more in the vitamin D section below.

Dosage Rationale: Strategic Effectiveness
