Free vs Total Testosterone

Total testosterone represents the combination of free testosterone and testosterone that is bound to either albumin or sex hormone binding globulin (SHBG). Free testosterone and testosterone bound to albumin are both considered to be bioavailable, meaning they are free to carry out various physiological functions. Whereas testosterone bound to SHBG is unable to actively do anything.

Thus, some males may get their levels tested by a physician and their total testosterone is in the normal range, yet they’re experiencing symptoms of low testosterone. This typically means that their free testosterone is lacking. On the contrary, a male could have low levels of total testosterone but not have any symptoms because he has plenty of free testosterone. Therefore, the key player in both scenarios is the amount of SHBG a particular individual has, because it can drastically skew lab results.

    Fenugreek has grown in popularity tremendously over the past few decades as studies have unveiled its health benefits. It’s used in a wide variety of supplements, but most notably, natural testosterone boosters. Multiple studies have shown that fenugreek supplementation is capable of enhancing strength, improving body composition, raising levels of free and total testosterone, increasing libido, and anabolic activity.[1,2]

    In one study from 2016, researchers divided 60 healthy males into two groups. One group received fenugreek and the other were given placebo. All participants underwent an 8 week resistance training program and data was collected at baseline, week 4, and week 8.[2]

    At the end of the study, the researchers found that the fenugreek group had a statistically significant increase in free testosterone with no reduction in total testosterone compared to the placebo group.[2] They also reported a significant increase in muscular endurance and reduction in body fat compared to the placebo.[2]

    Although more research is needed to fully determine fenugreek’s mechanism of action, the majority of studies hypothesize that it’s able to boost testosterone levels via the inhibition of aromatase and 5α-reductase enzymes.[3] Aromatase and 5α-reductase are two major enzymes involved in testosterone metabolism, one converts testosterone to estrogen and the other converts testosterone to dihydrotesterone (DHT). Therefore, being able to inhibit these enzymes is advantageous for increasing testosterone levels and keeping them elevated.

    Other benefits of fenugreek supplementation include improved insulin sensitivity, decreased LDL, VLDL, and triglyceride levels, and increased HDL cholesterol levels.[2,4]

  • PrimaVie (Shilajit extract) (50% Fulvic Acid with DBP Core Nucleus) (Dibenzo-a-pyrones (DBPs) Plus Dibenzo-a-pyrone Chromoproteins (DCPs)) – 250mg

    The next trademarked ingredient in Alpha-T is PrimaVie, a high quality, clinically studied, purified extract of shilajit from Natreon.[5]

    Shilajit extract contains a combination of dibenzo-α-pyrones (DBPs), DBP-chromoproteins (DCPs), fulvic acid and over 40 different minerals.[5-7] It’s been historically used in Ayurvedic medicinal practices to treat a variety of chronic diseases and is typically extracted from the Himalayas.[5-7] Today, PrimaVie is touted for its ability to support healthy testosterone levels, energy, endurance, healthy aging, collagen, and skin/bone health.[5-7] PrimaVie is standardized to contain at least 50% fulvic acid, which is one of the key components associated with its purported benefits.[5]

    Training is one of the best things you can do to naturally increase testosterone levels.

    In a landmark study from 2015, researchers recruited 96 participants to determine shilajit extract’s effects on male androgenic hormone levels.[6] Half of the participants were instructed to take a 250mg capsule of PrimaVie two times a day, while the other half were given a placebo.[6] After 90 days, the researchers found that the shilajit extract was able to significantly increase both total and free testosterone levels, while the placebo group actually saw a decrease.[6]

    They also reported that the shilajit group had a significant increase in follicle stimulating hormone (FSH), but no change in luteinizing hormone (LH) levels.[6] Both FSH and LH play a key role in testosterone synthesis.[6] Lastly, there were greater levels of DHEA with shilajit extract supplementation, which is a precursor to testosterone production.[6]

    One other notable human clinical trial reported that PrimaVie supplementation led to an upregulation in extracellular matrix (ECM) related genes associated with mechanotransduction, elasticity, repair, and regeneration within muscles.[7] Beyond those two human clinical trials, animal studies have shown an increase in endurance and energy levels by increasing adenosine triphosphate production.[7]

  • DIM (Diindolylmethane) – 100mg

    If you’re really looking to optimize your hormone levels, then diindolylmethane (DIM) is a great option. We are starting to see DIM added to a wide variety of supplements geared towards health and wellness, and for a good reason! Although Alpha-T is specifically formulated for males, both men and women can significantly benefit from supplementing with DIM.

    DIM is known for its unique capability of balancing estrogen levels by acting as an aromatase antagonist.[8] We discussed aromatase in the fenugreek section above, but just as a reminder, aromatase is an enzyme that converts testosterone to estrogen. Despite the misconception that males should try to lower estrogen levels at all costs, that couldn’t be further from the truth.

    As with anything in life, balance is key and estrogen plays a crucial role in both men’s and women’s health, though women just have a lot more of it. Because DIM is a potent aromatase inhibitor it can promote increased testosterone levels in males and keep estrogen levels from getting too high in females.[8]

    Studies also show that DIM may be able to inhibit estrogen absorption which is another way it keeps estrogen levels in check.[9] Lastly, DIM promotes a healthier ratio of “good” to “bad” estrogen by increasing estradiol 2-hydroxylase (EH) production opposed to 16-alpha-hydroxyestrone.[10]

    Overall, DIM is a phenomenal ingredient for both men and women and interestingly enough, it’s naturally occurring in cruciferous vegetables, including broccoli and cauliflower.[11]

  • Boron (as Boron Citrate) – 10mg

    Here’s a mineral that we don’t talk about much on the blog… boron! This trace mineral doesn’t get the spotlight very often, but it’s involved in several physiological processes and functions.

    Here are just some of the things why getting enough boron in on a daily basis is important: promotes growth and maintenance of bone, improves wound healing, has anti-cancer, anti-inflammatory and antioxidant properties, improves cognition, and supports the body’s utilization and production of estrogen, vitamin D, and testosterone.[12] That’s a fairly long list, but in regards to Alpha-T, the main one we are concerned with is testosterone production and utilization!

    Optimal hormone levels are crucial for progressing in the gym.

    One study in particular published in 2011 found that boron mineral supplementation significantly increased both plasma boron concentration levels and free testosterone levels.[13] Although they had quite a small sample size (only 8 participants), this was one of the first studies to show that boron supplementation could raise free testosterone levels.[13]

    As of writing this article, there are no other human clinical trials that we know of which assess boron’s effects on testosterone levels. Thus more research is warranted, however boron it’s clear that boron plays a key role in various aspects of health.[13] For example, boron has been shown to prevent vitamin D deficiency via extending the half-life and bioavailability of vitamin D.[13] It also significantly enhances magnesium absorption, which is another key mineral.[13]
